First and Last name of the athlete that will be placed on the shirt
Athletes Number
Name of the team (Arm sleeve/optional)
Let us know what sport type this is for so that we can place the "ball" on the front for design. (Example: baseball, soccer, basketball etc.)
 
*Please note* if you select the "T-Shirt Option" you will receive 2 shirts alike. 
For the T-shirt and Crewneck options, the name will be placed across the top back from shoulder to shoulder.
